As part of their commitment to continually try to achieve the highest standards, the staff at Roberts Health Center continuously monitor and evaluate their procedures to identify and close gaps. Over the past year, they identified an inconsistency in their antibiotic stewardship program. A quality improvement team was assembled, led by the Assistant Director of Nurses with consultation of staff nurses, physician, pharmacy, laboratory, and radiology staff.

Liberty Commons at Broadreach Health is located in the picturesque town of North Chatham, Massachusetts. Known as a “Community of Care on Cape Cod,” Liberty Commons is a rehabilitation and skilled care center with short-term inpatient programs and an outpatient rehabilitation clinic.
The Royal Megansett Nursing Home is located in scenic North Falmouth, Massachusetts, overlooking Megansett Harbor and Buzzard’s Bay. The facility is rated five stars and boasts 28 private rooms with third floor apartments for assisted living residents.
Royal Falmouth a 121-bed facility located in Falmouth, Massachusetts offers long-term care, short-term rehabilitation and memory care for residents with dementia. Within the facility, 23% of residents were using alarms, and in some cases, residents were double alarmed utilizing bed, chair or personal safety alarms to prevent falls or other injuries.
Staff from the Nursing and Recreation Departments at Apple Valley Center, a skilled nursing and rehabilitation center located in Massachusetts, were focused on improving their residents’ quality of life and sought to reduce the incidence of falls and falls with injury in their facility.
Wake Robin, a privately-owned nursing home in rural Vermont with less than 50 skilled residents, has been actively engaged with the New England QIN-QIO since 2014 to create and implement their Quality Assessment and Performance and Improvement Program (QAPI). Using these tools from their QAPI program, the leadership at Wake Robin initiated a Performance Improvement Project (PIP) and Root Cause Analysis to address challenges related to managing the disposal of Regulated Medical Waste. (RMW).
