Success Story IconAs part of their commitment to continually try to achieve the highest standards, the staff at Roberts Health Center continuously monitor and evaluate their procedures to identify and close gaps. Over the past year, they identified an inconsistency in their antibiotic stewardship program. A quality improvement team was assembled, led by the Assistant Director of Nurses with consultation of staff nurses, physician, pharmacy, laboratory, and radiology staff.

This new leadership at Hanover Terrace Health and Rehabilitation Center worked together to devise a plan that began to revolutionize the culture of the facility and drive down the antipsychotic usage rate.

Success Story IconBane Care Management LLC operates 14 Skilled Nursing Facilities, three Adult Day Health Centers and two Assisted Living Facilities in Massachusetts. The Bane Care facilities located on the South Shore of Massachusetts use the CoreQ patient experience survey, a set of five measures for skilled nursing care centers and assisted living communities to use to assess satisfaction among patients, residents, and their families. The Bane Care Management leadership team and frontline staff including admission, nursing, and social work noted a variance in survey results among the facilities and began to investigate the variance to improve their CoreQ score.

Five Star Nursing Home Windsor Health and Rehabilitation Center, LLC is a family-owned and operated 108-bed skilled nursing facility serving the Greater Hartford and Greater Springfield areas. Under new ownership since January 1, 2016, the facility offers a range of care options, including short term, long-term rehabilitation, and skilled nursing services which includes orthopedic, stroke, cardiac, pulmonary rehab, post-transplant care, wound management, and end-of-life care.
