Practice Transformation


The U.S. health care system is rapidly moving from a volume driven, fee-for-service model to multiple integrated care models and payment structures. Under the Transforming Clinical Practices Initiative (TCPI), the New England QIN-QIO is excited to collaborate with Practice Transformation Networks (PTNs) and Support and Alignment Network 2.0 (SAN 2.0) to help clinicians achieve large-scale health transformation.

Our goal is to assist approximately 35,000 clinicians in the New England region in the improvement of health outcomes for millions of Medicare, Medicaid and CHIP beneficiaries and other patients and reduce unnecessary hospitalizations for 5 million patients through practice assessments, tests and educational opportunities.
