WEBINAR: Highlights from Home – Innovative Approaches to Enhance Care Transitions and Medication Safety

Successfully transitioning a patient from an acute care setting back into the community continues to be a challenge. Research suggests that defined best practices can create a strong foundation for safe care transitions, as long as the right individuals are involved and that each provider’s role is clearly defined.
The New England Quality Innovation Network-Quality Improvement Organization (QIN-QIO) hopes you were able to join us on Thursday – January 25th (11am -12pm) to hear how two programs – Connecticut’s Home Health Readmissions Collaborative and Vermont’s SASH (Support And Services at Home) Program – exemplify this principle with their innovative approaches to enhance care transitions in the home setting.
After completion of this session, participants are able to:
- Describe how these two programs utilized available data and collaborative input to inform their efforts,
- Leverage the identified best practice tools to support home health care transitions, and
- Explain how long term services and support providers, such as SASH, can help enhance medication safety during transitions of care.
The Connecticut Pharmacists Association is accredited by the Accreditation Council of Pharmacy Education as providers of continuing pharmacy education. Pharmacists in attendance who complete an on-line evaluation can receive up to 1 contact hour of credit.
